Facility Placement.
Deciding on Facility Placement for a loved one can be a very difficult, delicate, and complicated process. Hospice & Homecare Advocacy understands these complexities and is able to effectively work with the interdisciplinary collaborative team (ie: Facility PCP, SW, Case Managers, etc.) to discuss safe options between facility placement vs. remaining at home with home care.
